Peter Attia's Four Horsemen: The Diseases That Actually Shorten Your Life
Dr. Peter Attia argues most people are optimizing for the wrong risks. His framework identifies four disease categories responsible for the vast majority of premature death.

The Four Horsemen
Attia's framework centers on cardiovascular disease, cancer, neurodegenerative disease, and type 2 diabetes and related metabolic dysfunction. His argument is that these four categories account for the vast majority of non-accidental deaths, and that the biological processes driving all four typically begin decades before diagnosis.
The Core Argument: Medicine 3.0
Attia distinguishes what he calls "Medicine 2.0," the conventional model focused on treating disease once it's diagnosed, from "Medicine 3.0," a model focused on identifying and addressing risk factors decades earlier, before disease is clinically detectable. This is closely aligned with the root-cause, functional-health framing we've written about elsewhere: addressing the underlying driver rather than waiting for the downstream diagnosis.
What He Emphasizes as Levers
Exercise, specifically VO2 max and strength. Attia has been particularly vocal that cardiorespiratory fitness (VO2 max) and muscular strength are among the most powerful predictors of both lifespan and healthspan in the research literature, often more predictive than many conventional risk factors.
Zone 2 training. A specific, moderate-intensity aerobic training zone that Attia argues builds mitochondrial density and metabolic efficiency more effectively than higher-intensity work alone, without the recovery cost.
Sleep and emotional health. Attia treats these as full pillars alongside nutrition and exercise, not afterthoughts, citing their downstream effects on all four horsemen.
Earlier, more aggressive risk-factor screening. Rather than waiting for standard reference ranges to flag a problem, Attia advocates tracking trends in relevant biomarkers over time and intervening well before a diagnosis would conventionally be made.
