Spike Protein Detox: Dr. McCullough's Base Protocol, What the Research Shows, and What to Know

Some people never feel fully like themselves after COVID-19 or vaccination, and functional doctors point to lingering spike protein as one reason. Here's what a spike protein cleanse includes, why Dr. Peter McCullough recommends nattokinase, bromelain, and curcumin, what the research shows, and the safety facts that matter.

Some people never feel fully like themselves after COVID-19 or vaccination: tired, foggy, or just off. Functional doctors point to one possible reason: the spike protein, the part of the virus that the vaccines also teach the body to make, can linger in some people for months.

That's the idea behind a spike protein detox, sometimes called a spike protein cleanse. Here's what it includes, why functional doctors use it, what the research shows, and what to know before you start.

At a glance

Research stageExperimental

Lab and animal studies plus personal stories; human trials still needed.

What it is

A supplement program, most often nattokinase, bromelain, and curcumin, that functional doctors use to help the body break down and clear lingering spike protein after COVID-19 or vaccination.

Why people choose it

For lingering symptoms after COVID-19 or vaccination, like fatigue, brain fog, and heart or circulation concerns. Dr. Peter McCullough’s “base spike detoxification” is the best-known approach.

What the research shows

In lab studies, nattokinase broke down spike protein, and bromelain with NAC took it apart. Some people carry spike protein in their blood for months. Trials of detox programs in people are still to come.

Know first

Nattokinase, bromelain, and curcumin all thin the blood, so check with your doctor if you take a blood thinner or have surgery coming up. Curcumin can affect thyroid and estrogen medicines.

Where to get it: Over-the-counter supplements, best chosen with a functional or integrative practitioner.

What It Is

A spike protein detox is a program of natural enzymes and plant compounds meant to help the body break down spike protein and calm the inflammation it can cause. The best-known version, Dr. McCullough's "base spike detoxification," uses three supplements:

  • Nattokinase, an enzyme from natto, a traditional Japanese food made from fermented soybeans
  • Bromelain, a group of enzymes from the pineapple stem
  • Curcumin, the active compound in turmeric

What Functional Doctors Say

Dr. Peter McCullough, an internist and cardiologist (MD, MPH), laid out the approach in the Journal of American Physicians and Surgeons in 2023. His reasoning:

  • Spike protein can linger. It has been found in the blood and in immune cells for months after infection or vaccination, and he sees it as a driver of inflammation and clotting in people who don't recover.
  • Enzymes can break it down. Nattokinase and bromelain are protein-digesting enzymes, and both broke down spike protein in lab studies.
  • Curcumin calms the fallout. It eases inflammation, and in computer models it blocks the spots where spike protein attaches to cells.

He calls the three together "a reasonable empiric strategy" for people with lingering symptoms after COVID-19 or vaccination, and he's candid that "no therapeutic claims can be made until large prospective randomized double-blind placebo-controlled trials are completed" (Journal of American Physicians and Surgeons, 2023).

Why People Choose It

  • Lingering symptoms: fatigue, brain fog, poor exercise tolerance, or heart and circulation concerns that started after COVID-19 or vaccination
  • Supporting the body's own clearing: helping the body break down and clear what it no longer needs
  • Natural and accessible: food-based enzymes and turmeric, available over the counter
  • Peace of mind for people who want to support their recovery

What the Research Shows

15 monthsafter infection, spike protein fragments were still found inside certain immune cells in people with long COVIDFrontiers in Immunology, 2022
0 of 45vaccinated young people without heart problems had free spike protein in their blood; it was clearly elevated in those with post-vaccine myocarditisCirculation, 2023
31%of people with long COVID had virus proteins, mostly spike, in their blood 6 to 12 months after infectionClinical Microbiology and Infection, 2026
  • Spike protein can linger. In a study of 46 people, fragments of spike protein were found inside a type of immune cell (non-classical monocytes) in people with long COVID up to 15 months after infection (Frontiers in Immunology, 2022). In teens and young adults hospitalized with myocarditis after an mRNA vaccine, researchers at Massachusetts General Hospital found free spike protein in the blood, while none was found in 45 vaccinated people without heart problems (Circulation, 2023).
  • How long it lasts: in a two-year study of 425 adults, virus proteins, mostly full spike, showed up in the blood of 31% of people with long COVID, 20% of people who recovered, and 5.4% of people never infected, 6 to 12 months after infection. By 18 to 24 months, they were rarely found, and they didn't match people's symptoms, so the researchers didn't recommend using them to guide treatment (Clinical Microbiology and Infection, 2026).
  • Nattokinase: in lab studies, nattokinase broke down spike protein in a dose- and time-dependent way, including spike on the surface of cells (Molecules, 2022).
  • Bromelain: bromelain combined with acetylcysteine (NAC) took apart the spike and envelope proteins and inactivated the virus in lab studies; neither one did it alone (Viruses, 2021).

What to Know Before You Start

Know first

  • All three thin the blood. Nattokinase, bromelain, and curcumin each affect clotting, and together the effect adds up. Talk with your doctor first if you take a blood thinner or aspirin, have a bleeding disorder, or have surgery or dental work coming up.
  • Allergies: skip nattokinase with a soy allergy and bromelain with a pineapple allergy.
  • Medicines: curcumin can affect thyroid and estrogen medicines, and bromelain can change how some antibiotics and other medicines are absorbed. Ask your pharmacist.
  • Pregnancy, breastfeeding, and children: there isn't enough information on these programs, so ask your doctor.
  • Get checked for heart symptoms. Chest pain, shortness of breath, or a racing or irregular heartbeat needs a doctor right away. Call 911 for severe symptoms.

We don't share doses or protocols. A practitioner who uses these programs can guide you.

How to Find a Practitioner

Functional and integrative doctors who work with long COVID and post-vaccine symptoms can test, tailor a plan, and watch for interactions with your medicines. The Natural Health Network directory can help you find holistic practitioners near you.

Questions to ask

  • Could lingering spike protein be part of what I'm feeling, and what else should we check?
  • Are nattokinase, bromelain, or curcumin safe with my medicines?
  • Which products and forms do you trust?
  • How will we know if it's helping, and when should we reassess?
  • Should I see a cardiologist for any of my symptoms?

Common Questions

Can I test for spike protein? Research labs have measured it, but it isn't a standard test, and the 2026 study found blood levels didn't match people's symptoms. A practitioner can decide what testing makes sense for you.

Can I just eat natto? Natto is the traditional food nattokinase comes from, and it's a nourishing fermented food. Supplements concentrate the enzyme.

Is this only for people who were vaccinated? No. Functional doctors use the same approach after COVID-19 infection, since both expose the body to spike protein.

Do I need a detox if I feel fine? Most functional doctors focus on people with lingering symptoms. If you feel well, the everyday foundations are the best support.
