Perimenopause Protocols: Weight, Hot Flashes, Sleep, and Feeling Like Yourself Again

Perimenopause changes how your body handles food, stress, heat, and sleep. Here's our root-cause approach to the stubborn weight, hot flashes, night sweats, and brain fog, with the modalities and habits that help most.

"I'm doing everything I've always done, and it's not working anymore." We hear it from women in their late 30s, 40s, and early 50s every week. The weight settles around the middle, sleep falls apart, hot flashes show up at the worst times, and energy and patience run thin.

You're not imagining it, and you're not failing. Your body is changing, and the approach needs to change with it. Perimenopause isn't something to just push through. It's a transition to support, and when you work on the root causes, most women feel dramatically better.

What's Really Going On

Perimenopause is the years leading up to your final period, often starting in your 40s and sometimes earlier. Estrogen and progesterone don't simply decline. They swing unpredictably, and those swings drive many of the symptoms. Progesterone, your calming, sleep-supporting hormone, often drops first.

Estrogen does far more than run your cycle. It supports insulin sensitivity, muscle, bone, blood vessels, your brain, and your body's thermostat. When it becomes erratic, you feel it everywhere:

  • Weight and shape change. The long-running SWAN study, which followed thousands of women through midlife, found that fat gain speeds up and lean mass declines during the menopause transition, with more fat shifting to the belly.
